Health Forms?

With summer almost upon us, don’t wait until the last minute to send us camp forms for completion. If at all possible, use the form you were provided at your routine health supervision visit. If the camp does not accept the form, we ask for at least 72 hours to complete your forms.

Influenza Vaccine

It’s that time of the year again. Influenza vaccines are available for all age groups. The current recommendations are that everyone over the age of 6 months should receive influenza vaccine on an annual basis.  Please call for an appointment. We have tried walk in clinics in the past and they just do not work
